Roger Erickson from Fire Ops. passed away July second 2010 please remember his family in your thoughts and prayers. Thank you Members of Local 2384 for your gift of $1,000 dollars to his family.

I would like to send a few thank you's to the guys at the hall, our newly remodeled hall.  Thanks to all who helped with the remodel.  
 
First of all, thank you for settling your disagreements when and where they should be settled; for representing the hall as a place of respect for one another!   
 
Thank you for the organization and dedication in our contract negotiations (furlough days would have sucked), and the 400 plus members who showed up to vote for our final contract.
 
A SPECIAL THANKS FOR SAVING OUR SHORTS!  Thanks Jimmy and Luis for filing the grievance that outlined two lies management tried to use to disallow the wearing of shorts. 
 
Thanks to everyone who is an elected official and to the people who donate their time and expertise to keep our hall what it is.

BENEFICIARY ARTICLE

 

                Provide for your family

 

Planning your retirement is more than just deciding how to spend your money. The choices you make now may allow you to provide for your family’s needs even after you’re gone.

 

Whether you’re participating in the city’s 457 Deferred Compensation Plan, the 401(a) Plan, or if you’re an active employee who is only participating through contributions made on your behalf by the city (Fringe contributions) to the 401(a) Plan, designating a beneficiary means you control who receives your benefits if you die before your account is paid out. Even an out-of-date beneficiary form could override wishes you express in a will.  To check or update your beneficiary:

·          Review your latest account statement (on the last page); or

·          Call Nationwide at 602-266-2733 or 1-800-891-4PHX (4749) to confirm your beneficiary information or to request a Beneficiary Designation Form;

·          Go to the 457 link to your online account on www.phoenixdcp.com; or

·          Print a beneficiary form from www.phoenixdcp.com under “Forms” and send your completed form to Nationwide.

 

As you may know, you can update your beneficiary designation(s) for your city life insurance by going online to the eCHRIS database; and you can update your beneficiary(ies) for your city pension by contacting the city’s Retirement Office at        534-4400 or Benefits Office at 262-4777 to obtain a beneficiary form.
